Form fitting - "nice fit"

Onefer... every occasion, whether you're coming ("just fill these out and someone will be with you shortly") or going ("please fill out this survey, easier her while your experience is still fresh").

Twofer... one deal, if you're also filling it out, assisting someone else, which may, despite looking the same, may require a few extra steps, like proving you're who you say you are, in relation to the person of interest.
